The teams of the Beaufort Volleyball Club meet on a regular basis at the Beaufort Boys and Girls Club on 1100 Boundary street, Beaufort, SC.

Beaufort Boys & Girls Club Application

In order to use the Beaufort Boys and Girls Club facility, please fill out a membership application form (under: Documents from 10/20/2012 BVC Info Meeting). Payment to the Boys & Girls Club will be made by BVC.

Notes to the membership application form:

Page 1: SS# & Insurance info is not required. Demographic info is requested to assist the Beaufort B & G Club in obtaining proper funding.

Page 2: item 1 & 2, Photo Permission & Report Card Permission info requested. Rest of page not applicable.

Page 3: Report Card Permission Request Form, to assist the Beaufort B & G Club in obtaining proper funding, permission requested.

Page 4: Please read and sign.

Please bring completed forms to BVC tryouts along with USAV membership application confirmation, Medical Release Form, and tryout fee.
