Most forms are now PDF Fill In Forms. You can type in most of the information on the form right from your computer! Once you have entered all the necessary information you can print it off, sign it, then mail or fax the form(s) to us. (You can also save the form to your computer for future reference or to complete at a later date. You will need to use the "save as" feature found under the "File" button.)

2009 Fall Camper Weekend Registration Form
Fall Weekend is a great way to enjoy camp and the friends you have met here. There will be a campfire on Friday night, activities during the day on Saturday and, of course, a dance on Saturday night. New campers are also welcome, so bring a friend! The cost is $60 per camper.

Camper Health History Form
The Camper Health History form must be turned in on the first day of camp with a photocopy of the camper's insurance card. DO NOT TURN IN THE Camper Health History Form WITH YOUR APPLICATION. A doctor's signature is required on the Camper Health History Form if over-the-counter or prescription drugs/medication are to be administered at camp.

2009 Dates
- Early Bird Discount of $20 per week if registered before April 15th.
- Multiple weeks may be requested and will be assigned based on availability.
- A separate application is needed for each week.
- Campers are not permitted to stay at camp over the weekend. They must go home on Friday and return on Sunday.
Drop-Off and Pick-Up Times
Registration is Sunday from 1:00 p.m. to 3:00 p.m. at the Main Lodge. If you are going to be late, please call us at camp: (585) 535-7381. Your child’s first meal at camp, supper, will begin at 5:30 p.m.
During Weeks #1-5 The last meal is Friday’s supper. You must pick up your child between 7:00 p.m. and 7:30 p.m. Friday. Campers remaining at camp after 8:00 p.m. will be assessed a $50 late fee.
During Week #6 the last meal is Friday’s breakfast. You must pick up your child between 9:00 a.m. and 9:30 a.m. Friday. Campers remaining at camp after 10:00 a.m. will be assessed a $50 late fee.
Campers who are staying for another week must leave on Friday and come back to camp on Sunday. There are no staff members available at camp over the weekend.
Prepaid Fees
The following fees must be submitted with the Camp Registration Form:
- Deposit - $50 - Non Refundable/Non Transferable
- Horsemanship Activity Fee - $95 - (If Participating in this Activity)
- Specialty Camp Fee - $25 - (If Participating in a Specialty Camp)
- Horse Camp Fee - $50 - (If Participating in Week #1 Horse Camp)
Deposits
A non-refundable, non-transferable deposit of $50, must accompany your application for each week at camp. A separate application is required for each week of camp and for each camper. If using a Wyoming County Cookie Sale Campership, which reduces the balance to less than $50, pay the balance only.
Returned checks will be charged a $25 fee. This fee, in addition to the amount of the returned check, must then be paid in full by money order or credit card.
The balance due for camp must be paid in full three weeks before your camper comes to camp. Failure to do so may result in the loss of your reservation for camp!
Cornell Cooperative Extension of handles all registrations.
Camp Refund Policy
Deposits:
Deposits are non-refundable and non-transferable (a deposit cannot be applied to the balance of another week’s camp fees, nor transferred to another camper’s fee).
Before the start of camp:
- If a doctor’s certificate is provided stating that the camper is unable to attend camp because of illness or injury, the camp fee, minus the deposit, will be refunded.
- A letter from a principal or other school official is required for campers who cancel out of a week because they have to attend summer school. The balance, minus the deposit, will be refunded.
- Refunds for the balance of the camp fee, less the deposit, are made ONLY for cancellations received in writing at least three weeks prior to the opening day of the camp session.
During Camp:
- A camper who does not arrive, arrives late, leaves early, or attends only part of the session will not receive a refund.
- If the camp nurse and the camp director decide that a camper should be sent home due to medical reasons, a refund for the balance of the camp period will be made on a prorated basis, minus deposit, computed as of noon each day.
- If a camper is experiencing a difficult adjustment to camp or is sent home because of a behavior problem, no refund will be granted.
Refunds will not be issued until after August 15, 2009
