Basketball Info

• HS Summer Open Gyms  9-11am Tuesday & Thursday

• HS Varsity Summer League-Wednesday nights @ Hempfield

• HS JV Summer League-Monday nights @ Hempfield

• JH Summer League-7th grade & 8th grade (both @ Hempfield on Monday nights)

CVSD sports physicals for the 2012-13 school year will be held Wednesday, June 6, from 6:00 to 9:00 p.m. This is the only time that the school will offer sports physicals for the upcoming school year. There is no make-up date.

To avoid lines, we recommend the following arrival times, although families with children in several grades are welcome to come at the same time.

• 6^th grade: 6:00 p.m.
• 7^th grade: 6:30 p.m.
• 8^th grade: 7:00 p.m.
• 9^th grade: 7:30 p.m.
• 10^th grade: 8:00 p.m.
• 11^th grade: 8:30 p.m.

Limited appointments will also be available from 3:00pm to 5:45 p.m. To schedule an appointment, call the athletic training room at 397-5231 x2307.

The cost this year is $20.00, cash or check written to CVHS.

Students who participate in the school physical are eligible to have the team physician sign their working papers, driver’s physicals, and camp forms throughout the following school year. This physical is also acceptable for the state-mandated 11^th-grade physical; copies of all juniors’ physicals will be given to the school nurse.

-  The sports physical must be completed no earlier than June 1

-  The one-page CV Sports Medicine Information form MUST be included with all athletes’ sports physicals

-  The full sports physical form is now CIPPE Sections 1-5; print only the first five pages

-  The parent re-certification form is now CIPPE Section 6; print only page six

-  CV’s athletic training staff will notify any individual who needs the doctor re-certification form (CIPPE Section 7)

**Both physical forms can be printed from the Important Documents section.**
