Trying out for the 2013 - 2014 Season?
If you are planning on trying out for the 2013 - 2014 volleyball team, MDUSD requires that the Athletic Activity Clearance form be completed by a physician (not a chiropractor) between July 1st, 2013 and prior to the first day of tryouts or practice.
Without this form, access to tryouts/practice will not be allowed.
